Intensive use of mobile phones has raised concern about possible health problems caused by Radio frequency electro magnetic waves emitted by these devices
A new study (sept 2008) from Cleveland clinic showed that if Men keep their phones on and in their front pockets the mobility and viability of their semen could be adversely affected. Anu Gokhale, technology professor at ISU said that studies have found the harmful effect of electromagnetic radiation given out by cell phones on body tissues.
The Cleveland clinic involved 32men in their study. Semen sample collected, divided into 2 parts. One part of sample exposed to electro magnetic radiation emitted from commercially available cell phones with 850 MHz frequency keeping it at a distance of 2.5cm from sample, the other part kept as control.
Researchers found an in increase in seminal oxidative stress resulting in decreased sperm motility and viability. As the fertilization of an egg depends on how long the sperm survives and how mobile it is, decrease in motility and viability of a sperm lessens its ability to fertilize an egg.

DR RAMA RAJU & DR KAVITHA

Polycystic ovary syndrome (PCOS) is a heterogeneous endocrine disease affecting about 5–10% of women of reproductive age One of the most important and newly discovered features of the syndrome is insulin resistance (IR), which is constantly accompanied by compensatory hyperinsulinaemia. Despite the frequent presence of obesity in PCOS patients and the wellestablished relation between obesity and IR, a specific mechanism of IR in PCOS, independent of obesity and consisting of a specific post-receptor defect, has been demonstrated. The rationale to use insulin sensitizers for ovulation induction in infertile PCOS patients is based on these findings.
The first study showing the beneficial effects of insulin sensitizers on ovarian function was published in 1994 by Velasquez and co-workers (Velasquez et al., 1994). They demonstrated that metformin at doses of 1500 mg daily is effective in reducing body weight, improving menstrual cycles and fertility in PCOS women. Following this study, there was a dramatic increase in the number of publications concerning the use of insulinsensitizers and PCOS.

Outpatient hysteroscopy and subsequent IVF cycle outcome: a systematic review and meta-analysis A study pblished in RBM online published by “Assisted Conception Unit, Guy’s and St. Thomas’ Hospital NHS Foundation Trust, London”demonstrates a beneficial effect of hysteroscopy performed prior to ovarian stimulation on the outcome of the IVF cycle in patients having their first or subsequent IVF attempt. This positive impact on IVF outcome could be related to the ability of OH to reliably detect and potentially treat intrauterine pathologies encountered during the procedure such as intrauterine adhesions, endometrial polyps, submucous fibroids, endometritis or uterine malformations that could interfere with implantation . These pathologies have been shown to be present in up to 50% of infertile patients and the risk of their presence increases with increasing number of IVF failures .. The prevalence of intrauterine pathology encountered at hysteroscopy in the five studies included in this systematic review ranged from 25% to 56%. Correction of most of these abnormalities and restoration of a normal uterine cavity is possible during OH with reported good outcome, similar to that achieved in patients with a normal hysteroscopy In conclusion, office hysteroscope might be associated with improved IVF outcome when performed immediately before commencing an IVF cycle.

Overweight and obesity has reached epidemic proportions worldwide.) Data collected by the National Center for Health Statistics indicate that the prevalence of obesity, defined as a body mass index >30 kg/m has increased from 12.8% in 1976-1980 to 22.5% in 1988-1994 and 30% in 1999-2000. In an effort to increase public awareness of the epidemic proportion of obesity, the Surgeon General has issued a call to action to prevent and treat overweight and obesity and their associated health complications.
obesity ,particularly morbid obese (BMI 40 or more) are associated with increased risk for hypertension, diabetes, hyperlipedemia ,sleep apnoea ,coronary heart disease and stroke. The cause of obesity is complex and poorly understood, but includes genetic, behavioral, psychological and other factors. Familial suggest that hereditary may explain 67% of the population variance in BMI. Decline in physical activity and increase in consumption of energy dense food may play a role
Bariatric surgical procedures reduce caloric intake by modifying the anatomy of gastrointestinal tract. They are two types ,restrictive procedure limits intake ,malabsoptive procedure by passes the absorption area of intestine .Roux-en-y gastric bypass is a combination of restriction ad absorption procedure. This procedure accounts for 92% of the bariatric surgery
Person with BMI >40 or BMI of 35 or higher with high risk condition like severe sleep apnea, obesity related cardiomyopathy are ideal for bariatric surgery The potential benefit of surgery in BMI of 30 to 35 remains unclear .Before taking up for bariatric surgery excluding secondary cause of obesity like hypothyroid, Cushing syndrome.and medication history of antidepressant, oral pill oral hypoglycemic agents should be obtained.Medi cal assessment for obesity associated condition and a psychological evaluation should be done.
A typical weight loss of 20 to 50 kg’s occurs with bariatric procedure. In general malabsorptive procedure have higher weight loss
Bariatric surgery is associated with death in 0.5% to 1.1% and common cause are pulmonary embolism and anastomostic leak..The post operative complication can be nausea and vomiting ( common ) dumping syndrome and vitamin deficiency.
